Once you have completed the creation of a Dynamic Form, you will want to link it on a webpage or publish it through email. Use the instructions below to obtain a link to your form that can be published.

Toner supplies can be ordered through Connecticut Business Systems (CBS). Departments do not get charged directly for consumable supplies when ordering. There are three options:
  
You cannot publish a link obtained from your browser's address bar to a Dynamic Form.

*1. Call 1-800-842-0009. Please have your equipment ID or serial number available to give to the supply team

Log into Dynamic Forms using the Portal link

*3. Go to [[www.cbs-gisx.com www.cbs-gisx.com]], Click support, then go to order supplies and fill out the information requested like screenshot below:

Locate the form for which you would like a link.

Highlight and copy the appropriate link.

The "Single Sign On" version of the form should be used for any forms distributed to MCLA students or employees who have an MCLA ID and account.
